Key Responsibilities:
Clinical Assessment and Treatment Planning:
Conduct in-depth assessments of clients' behavioral health and emotional needs.
Develop personalized treatment plans with the client and their caregivers, focusing on evidence-based therapeutic interventions.
Provide individual, group, and family therapy as well as home/community-based services to meet treatment goals.
Home and Community-Based Services:
Offer flexible service delivery in clients' homes, including evenings and weekends.
Support clients in developing essential life skills, social skills, and coping mechanisms.
Collaborate with families and caregivers to reinforce goals and improve outcomes.
Crisis Intervention:
Respond immediately to crises, providing stabilization and immediate support.
Develop crisis safety plans and strategies to prevent future crises.
Coordinate with crisis service providers and other professionals to ensure continuous care.
Case Management and Coordination:
Build relationships with community agencies, healthcare providers, and schools to support comprehensive care plans.
Coordinate services with all involved agencies and monitor client progress.
Collaboration and Teamwork:
Lead the HCT services in the community as part of the Children’s Behavioral Health Home (BHH) team.
Work collaboratively with a multidisciplinary team to ensure the best outcomes for clients and families.
Documentation and Compliance:
Maintain accurate, timely records of assessments, treatment plans, and progress notes.
Ensure compliance with all legal and regulatory requirements, and document billing promptly.
Physical & Work Environment Requirements:
Must be able to work in various environments, including homes and communities, where conditions may be noisy or chaotic.
Ability to travel to clients within the catchment area.
Regularly required to sit, stand, walk, talk, and hear.
Schedule:
This is a Full-Time exempt position

Requirements
Master's degree in a related field from an accredited college or university.
Licensure as LMSW-CC, LCPC-CC, LCSW, or LCPC in the State of Maine.
Valid driver’s license and reliable transportation are required.
Experience with behavioral health, crisis intervention, and case management is preferred.
