- Determines medical level of care for program eligibility; Coordinates and evaluates health resources necessary to meet individual or group needs.
- Evaluates the quality of program services. Educates the community and members of the health care team regarding health care needs.
- Supervision and personnel management of assigned HCMN team; Training and education of staff to roles and on policy, procedures, and protocols.
*Preferences may be given to applicants with two years of experience working with the ADvantage, State Plan Personal Care or Nursing Facility eligibility and with a working knowledge of Harmony, MMIS, PASRR, and IMS systems.
