Description

SUMMARY:

Under the direction of the Director of Health Services, provide individualized nursing care to participants enrolled in the Adult Services Program. Develop plans for care for all Adult Day Health Care (ADHC) participants. As a licensed practical nurse, you will be responsible for planning, organizing, and developing all nursing activities. In addition, you will be responsible for complete management, which includes developing, coordinating, and monitoring all nursing care for our participants, as well as conducting medical trainings for the Program Aids/CNAs/Health Advocate. Responsible for the managing and monitoring of all health-related requirements for Early Learning Services.

DUTIES AND RESPONSIBILITIES:

ADHC

  • Review Statement of Medical Status (SMS) information and request updates from physician annually and or as needed.
  • Assess each participant’s needs for ADHC care prior to admission and on an ongoing basis; perform assessments in the home as appropriate.
  • Complete all Nursing Intake forms for new participants entering ADHC program.
  • Revise all discharge summaries and ISPs as needed for ADHC participants.
  • Maintain the program’s plan for the self-administration of medication which is individualized for each participant as needed.
  • Review participants’ medications on a monthly basis for appropriateness; determine the need for laboratory monitoring, referrals to the attending physician for such tests and the effectiveness of the drugs prescribed.
  • Medication administration.
  • Monitor medication storage and record keeping.
  • Ensure that all medical records are maintained in a secure manner.
  • At least monthly, monitor and ensure the documentation of all medical services delivered pursuant to the care plan. Monitor vitals monthly.
  • Works with the consultant Nutritionist and participants’ physicians for assignment of diet
  • Maintain current physician’s orders for each ADHC participant.
  • Serve as coordinator and advocate between the participant and medical resources including the treating physician and the nutrition consultant.
  • Coordinate participant care education with caregivers as appropriate
  • Provide quarterly in-service training sessions to participants and/or caregivers on health-related matters or as needed.
  • Provide monthly health awareness trainings to the participants and/or caregivers or as needed.
  • Provide supervision, mentorship, and/or assistance to the Program Aides/CNA’s ensuring the completion of all tasks as needed. Provide coaching and support to the Aides.
  • Interact with and provide supervision/observation of participants.

Early Learning Services

  • Guide the Health Team with ensuring the program adheres to the health requirements and developmental screens within each child’s 45 days of entry into the program and assists the Health Team with completing on-going screenings for all sites.
  • Reviews all children’s health plans on a quarterly basis.
  • Conducts all staffings for children with health concerns.
  • Ensures accuracy in medication administration and documentation
  • Assists the Director of Health Services in all health-related trainings for staff and participants
  • Provides training and technical assistance to staff based on assessment of health, health and safety and nutrition service area needs
  • Monitors and ensures accuracy of health information in the ChildPlus.Net program (data management program).
  • Supervises Health Advocate
  • Performs all other duties as assigned within the scope of the job description.

Requirements

REQUIRED SKILLS AND EXPECTATIONS:

  • Ability to work cooperatively with all members of the staff.
  • Ability to obtain and organize resources that meet the needs of elderly and disabled participants.
  • Ability to train participants, caregivers, CNAs, and staff in health related matters.
  • Demonstrates an understanding of the dynamics of poverty, oppression, sexism, racism, ageism and other forms of societal and individual prejudice that confront people daily.
  • A proven ability to work effectively and respectfully with a broad range of people from diverse cultural, ethnic and social-economic backgrounds.
  • Ability to engage and motivate program participants to increase/maintain their self-reliance and work toward resolution of the challenges they face.
  • Ability to advocate on behalf of program participants and demonstrate a belief that participants have the capacity to grow and change.
  • Demonstrates a philosophy and values consistent with the purpose and goals of Kingsley House, utilizing strengths and empowerment perspective toward increasing the capacities of program participants and their families.
  • Possesses the ability to work holistically, as part of a team, with staff, program participants, their families, volunteers and the community at-large toward successful outcomes for program participants.
  • Respects the integrity of each program participant, their right to privacy and confidentiality and their right to participate to their maximum capacity in determining their participation in our programs, including the formation of a case or treatment service plan.
  • Maintains confidentiality with respect to participant, personnel and proprietary agency information.
  • Able to obtain and maintain Adult CPR and First Aid certification.
  • Able to meet deadlines, well organized, and able to work under pressure of multiple deadlines; demonstrates flexible and efficient time management and ability to prioritize workload.
  • Acts as a role model within and outside of the agency
  • Adheres to agency policies and procedures
  • Utilize Kingsley House email system to check and reply to messages on a daily basis in order to ensure the timely reception of important communications.

JOB REQUIREMENTS:

  • Valid Louisiana driver’s license.
  • LPN in the State of Louisiana.

EDUCATIONAL & EXPERIENCE REQUIREMENTS:

LPN certification required. Three to five years’ experience working in an accredited, licensed medical facility.
