Description

Job Title: HIM Technician FT M-F 8am-4:30pm

Supervised by: Director of HIM

Job Summary:  Responsible for the receipt, scanning and indexing of medical records documents into the hospital’s electronic medical record (EMR) in a timely and accurate manner. Reviews medical records post discharge for completeness in compliance with the hospital’s bylaws, accrediting organization standards, federal and state requirements. Assures that medical record deficiencies are correctly entered into the medical record deficiency system. Processes requests for copies of medical records following applicable state and federal guidelines. Prepares an accurate and complete birth/Fetal Death certificate.  Demonstrates Competency in the Following Areas:Collects all discharged patient medical records from the patient care units on a daily basis. Assembles, scans and indexes pertinent medical record documents into the EMR within 24 hours whenever possible. Combines current patient medical record with any previous patient discharges for a unit record. Analyzes medical records of discharged patients for completeness and accuracy according to department and hospital policy and procedures. Enters medical record deficiencies into EMR for clinician completion. Knowledge of medical record completion requirements for all patient types. Acts as a backup to notify clinicians of their incomplete/delinquent medical records Completes birth/fetal death certificates within 24 hours when possible and secures all required signatures for proper filing with local and state agencies. Adheres to hospital requirements, policies and standards on confidentiality. Process release of information requests for protected health information (PHI) for all request types, e.g. subpoenas, court orders, payors, patients etc. Answers telephone and assists both internal and external customers with requests for medical records.  Transcribes voice messages in an accurate manner. Must meet productivity and accuracy requirements for all clerical functions at a minimum of 95%. Maintains a good working relationship within the department and other departments. Performs additional duties as assigned by the HIM Lead Technician and/or Director of HIM. Supports and maintains a culture of safety and quality. Professional Requirements: Adheres to dress code, appearance is neat and clean. Completes annual education requirements. Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards. Maintains patient confidentiality at all times. Reports to work on time and as scheduled, completes work within designated time. Wears identification while on duty, uses computerized punch time system correctly. Completes in-services and returns in a timely fashion. Attends annual review and department in-services, as scheduled. Attends 12 staff meetings annually, or reads and returns all staff meeting minutes. Represents the organization in a positive and professional manner. Actively participates in performance improvement and continuous quality improvement (CQI) activities. Complies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the hospital, as well as the focus statement of the department.

Requirements

 Regulatory Requirements:

High School graduate or equivalent.

One year of hospital HIM experience preferred.

Language Skills:

Able to communicate effectively in English, both verbally and in writing.

Additional languages preferred.

Skills:

Basic computer knowledge.

Physical Demands:

Regularly required to sit and stand for long periods, walk, stoop, reach, crouch, bend, push, pull, and repetitive hand motion. This job requires lifting up to 25 pounds and ability to carry up to 20 pounds. Intermittent sustained or moderate physical effort required.
