Appleseed Community Mental Health Center is looking for a compassionate, organized, and community-minded Health Navigator to help individuals affected by substance use and co-occurring behavioral health conditions overcome barriers to care.

This position is a great fit for someone who enjoys building trust, connecting people with resources, and supporting individuals on their recovery journey. You’ll work directly with clients, healthcare providers, and community partners to improve access to treatment, recovery services, and other essential supports.

What you’ll do

• Help clients identify and address barriers to treatment, recovery, and other essential services
• Connect clients with behavioral healthcare, medical care, social services, and community resources
• Facilitate referrals and warm hand-offs to help clients successfully connect with providers
• Support access to peer recovery services for individuals affected by opioid use, stimulant use, and co-occurring behavioral health conditions
• Provide person-centered support that respects each client’s goals, culture, background, and individual needs
• Collaborate with healthcare providers and community organizations to coordinate services and strengthen support
• Promote substance misuse prevention and awareness of available recovery resources
• Maintain accurate, timely client records and documentation
• Prepare monthly reports on services provided, client progress, and outcomes
• Provide services in a variety of settings, including clients’ homes, public spaces, hospitals, and offices

What we’re looking for

• High school diploma or equivalent
• Strong written and verbal communication skills
• Ability to build trust and rapport with individuals experiencing substance use and behavioral health challenges
• Knowledge of local substance use treatment, recovery services, and community resources
• A respectful, nonjudgmental approach to working with people from diverse cultural and socioeconomic backgrounds
• Strong organizational and time-management skills
• Ability to work independently and collaboratively
• Basic proficiency with word-processing software and case management systems
• Professionalism, sound judgment, and a commitment to confidentiality

• Valid Ohio driver’s license
• Reliable transportation and personal auto liability insurance
• Flexibility to work some evenings and weekends to meet clients’ needs
• Ability to travel for client services, training, conferences, and other professional activities
• Current CPR, First Aid, and physical/verbal crisis intervention and de-escalation training
• Completion of Annual Motivational Interviewing Training
• Ability to operate basic office equipment and lift up to 20 pounds
• Successful completion of required background check and pre-employment drug screening
