The Health Service Coordinator assists the Nursing department with the coordination of individual healthcare needs and support systems that provide consistent, timely and appropriate medical care.  The Health Service Coordinator is not a licensed medical person, is not permitted to provide medical advice, and will act as a liaison and support to the Dungarvin New Mexico team.

What You Get to Do:  

• Support the Nursing department in the coordination and follow up needs associated with healthcare appointments
• Attend medical appointments when requested
• communicate all pertinent information after an appointment to the Nurse, Program Director, and Residential Coordinator.
• support the Nursing Department in maintaining paperwork, organizing and filing medical information as assigned
• Assist and coordinate with the pharmacy delivery of individual’s medications and pick up of medications by staff at the office
• monitor and maintain the Medication Administration Records (MAR’s).
• Build relationships with persons served and teammates. 
• Attend special community occasions and fun activities like outdoor walks, games, and social events. 
• Transport persons served to appointments 
• Empower people with disabilities to live life to the fullest. 

QUALIFICATIONS:

• Education: High school diploma or GED Certificate.  
• A minimum of two years of experience in the field of developmental disabilities, or a Bachelor's degree in a related field.
• Ability to exercise sound judgment in complex situations.
• Proven track record of effective teamwork to maintain consistency and quality in service delivery.
• Ability to assist in maintaining efficient office operations and possess skills in the area of financial management and/or scheduling.
• Must be proficient in Microsoft Word, Excel, Outlook and PowerPoint, as well as general office technologies
• Personal qualities should include confidence, flexibility, self-directedness, and the ability to maturely deal with unexpected demands and incidents.
