Job Description - Health Services Coordinator

JOB PURPOSE 


The Health Services Coordinator plays a key role in ensuring the overall health and well-being of the individuals we serve by working closely with the Community RN, direct support professionals (DSPs), case managers, and healthcare providers. In this role, you will assist with health-related coordination, ensure compliance with medical protocols, support medication management, and provide training to staff on health and safety practices.


 


POSITION TYPE


Hourly – Full-Time



MAIN DUTIES AND RESPONSIBILITIES



  • Health Coordination & Medical Support

    • Assist the Community RN in conducting health assessments and implementing individualized healthcare plans.

    • Facilitate medical appointments, coordinate transportation, and ensure proper documentation of medical visits.

    • Support DSPs and case managers in monitoring and reporting changes in individuals' health conditions.

    • Maintain up-to-date and accurate medical records, incident reports, and compliance documentation.

    • Ensure health-related needs and services are carried out in accordance with MO DMH regulations.



  • Medication Oversight & Compliance

    • Assist in the management of medication administration, ensuring compliance with Level 1 Medication Aide (L1MA) requirements.

    • Monitor medication records, ensure proper storage, and coordinate refills as needed.

    • Conduct medication audits in collaboration with the Community RN to ensure accuracy and compliance.



  • Staff Training & Development

    • Provide training and guidance to Direct Support Professionals (DSPs) on medical and health-related topics, including:

    • Recognizing and responding to medical emergencies

    • Infection control and universal precautions

    • Safe medication administration

    • Specific health concerns related to individuals with disabilities

    • Act as a resource for staff and families on healthcare-related questions and concerns.



  • Regulatory Compliance & Quality Assurance

    • Assist in ensuring all health-related policies, procedures, and documentation comply with MO DMH regulations and best practices.

    • Support the Community RN during MO DMH audits and inspections related to healthcare services.

    • Advocate for quality healthcare services for individuals with disabilities.



  • General Responsibilities

    • Communicate effectively with medical providers, families, and interdisciplinary team members.

    • Respond to health-related concerns and emergencies promptly and professionally.

    • Perform other duties as assigned.





QUALIFICATIONS & SKILLS




  • Must be 18 years of age or older.

  • High School Diploma or GED required.

  • Minimum 2 years of experience in healthcare, social services, or a related field.

  • Ability to coordinate medical care and collaborate with healthcare professionals.

  • Excellent documentation and organizational skills.

  • Strong communication and training abilities.

  • Must successfully complete a pre-employment background check and any subsequent background check after employment.

  • Must have a valid driver’s license and be able to obtain a Missouri Class E chauffeur’s license within 14 days of hire.

  • Successfully complete employment-related training as directed.

  • Ability to work empathetically with individuals with disabilities.

  • Ability to respond to changing health needs and situations.


 


PHYSICAL REQUIREMENTS



  • Ability to stand, walk, and move between different locations-program locations, community events, appointments, etc.

  • Ability to lift and move up to 50 pounds (e.g., assisting with mobility).
