Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. The Health Care Navigator contributes to the mission and values of Maple City Health Care Center by supporting patients and non-patients in accessing affordable health care.
Duties and Responsibilities
Assists patients and non-patients in accessing insurance information and coverage through Medicaid and the Marketplace
Helps patients access health care financial assistance programs through Goshen Health
Participates in community events to educate people about MCHCC’s navigator services
Maintains a high level of accuracy and integrity with documentation
Documents patient contact using electronic medical record
Utilizes telephone encounters and encounter notes for documentation
Follows up with care team as needed
Other duties that support our shared work, as assigned
Requirements
Required Skills and Abilities:
Experience working with diverse groups of people
Ability to take initiative, work independently, and collaborate with the team
Ability to respectfully translate a variety of real-life situations into the Medicaid framework
Fluently bilingual: English/Spanish; oral and written
Effective interpersonal skills
Education and Experience:
Associate degree strongly preferred, especially in social work or related field
Certification as an Indiana Navigator
Requirements:
Ability to perform prolonged periods of time in front of a computer
