Healthy Start Community Consortium Coordinator/Evaluator

JOB SUMMARY:

The Community Consortium Coordinator/Evaluator is responsible for evaluation activities regarding the Healthy Start Program, as well as in overseeing the Community Consortium and ensuring representation of members across the communities/populations served. The Community Consortium Coordinator/Evaluator will also assist in the implementation of plans aimed at reducing disparities in perinatal health outcomes, including care coordination for children, adults, and families within the Healthy Start Program.

 DUTIES AND RESPONSIBILITIES:

1. Responsible for collection, input and tracking client data/outcomes and conducting periodic reviews of data to ensure accuracy and for reporting purposes.
2. Provide interim data and information derived from surveys and tracking data to the Program Director on a regular basis; provide guidance regarding feedback process structure.
3. Provide the Project Director with annual evaluation reports, to be submitted to the Health Resources and Services Administration each year of the project, as well as a final report at the end of the five-year period of performance. Reports will compare actual and projected progress, and detail any lessons learned.
4. Plan, coordinate, and attend all Community Consortium meetings; recruit community members to join consortium.
5. Engage Community Consortium members to actively participate in the implementation of community driven approaches and activities to achieve work plan goals/objectives.
6. Manage Caseload of Healthy Start Participants in various counties as needed/ assigned.
7. Interview clients' families; complete intake and risk assessments which identify specific needs of each client and family.
8. Provide program updates and benchmarks at meetings to ensure meeting program goals.
9. Complete and maintain database according to Healthy Start guidelines.
10. Participate in staff meetings, quality assurance/evaluation activities, in-service, and all training as necessary and/or assigned.
11. Performs other duties as assigned.

 SUPERVISORY RELATIONSHIP:   Works under the supervision of the Healthy Start Project Director; supervises no other employees.

 QUALIFICATIONS:  A viable candidate for this position will have the following qualifications:

1. Bachelor's degree from an accredited college or university; relevant work experience may be considered in lieu of the educational requirement
2. Experience in both quantitative and qualitative data collection and analysis;
3. Skilled in database management and use;
4. Experience in a collaborative evaluation setting where program staff participate in designing conducting, and interpreting the evaluation;
5. Skilled in clear, jargon-free oral and written communication;
6. Strong record of completing reports on time and with a high-quality level of work;
7. Knowledge of community resources available/understanding of area community needs;
8. Must have reliable transportation, valid Driver's License, and auto insurance coverage;
9. Ability and willingness to work at times other than regular office hours;
10. Must be able to travel when necessary.

 MENTAL AND PHYSICAL DEMANDS/ENVIRONMENTAL FACTORS:

1. Ability to communicate effectively verbal/written), maintain emotional control under stress;
2. Constantly must be able to sit/stand in a stationary position 50% of the time.
3. Frequently must be able to move about inside office to access file cabinets, office machinery, etc.
4. Frequently must operate a computer and other office productivity machinery such as a calculator, copy machine, and computer printer.
5. Frequently must communicate with staff and others who have inquiries about the Healthy Start Project. Must be able to provide and discuss accurate information in these situations.
6. Occasionally must move/carry equipment weighing up to 30 pounds or more as needed.
7. Frequently must visually detect highway signs/interact with traffic while operating a vehicle.
