- Ongoing coverage
- 3 days/week with

- 1–2 weeks on/off possible)

• Shift Schedule: 8:00 AM – 5:00 PM

• Patient Demographics: Adult

• Patient Volume: 15 PD

• Tumor Board Participation

• % of Practice: 60% Hematology / 40% Oncology

• Physician in Practice:
- 3 Physicians at Hazard

- Solo Provider Coverage in Martin

• Support Staff:

- 1 APP Daily Oncology

- Trained RNs

- APP Support

• Specialty Backup: Hazard Physician Team

• Case Mix:

- Hematology and Solid Tumor Cases;

- High -Acuity Leukemia Cases Referred to Tertiary Centers

• Procedures: Infusion Supervision, Oncology Follow -Ups, Supportive Care Visits

• Trauma Level: Critical Access Facility

• EMR: Meditech

• Other Info:

Locums providers participate in tumor board
Approximately 20% new patients / 80% follow -ups
APPs assist with documentation and orders collaboratively
Standard oncology infusion supervision model
Commute bonus may be available; inquire with AM
• Hospital Privileges available: Yes
• Temporary Privileges available: Yes
• Travel, lodging, and malpractice insurance covered

• Active KY License or Willing to Obtain

• Recent Patient Facing Experience Required
