Job Description - Hepatic Artery Infusion Program Nurse Navigator

Massachusetts General Hospital Hepatic Artery Pump Program Nurse Navigator

Under the general supervision of the Nurse Director the Nurse Navigator works with the Hepatic Arterial Pump program to provide support of the daily clinic operations which includes direct and indirect patient care. Conveys a positive and professional image and represents the staff and the patients for in a professional, courteous and efficient manner. Responsible for facilitating a multi-disciplinary approach to achieve optimal outcomes and supports the mission, philosophy, standard, goals and objectives of MGH and the Cancer Center.

Nurse Navigator: demonstrates awareness & recognition of the operations providing support to clinic staff by assessing and evaluating needs of the program. Assumes key role in assuring and facilitating patient care activities for individuals preparing for Hepatic Arterial Infusion Pump, by coordinating between medical and surgical oncology, obtaining relevant medical records, referring and scheduling patients to new patient consultations as well as infusion center appointments, clinic visits, imaging and support services as appropriate.

Facilitates the coordination of care and demonstrates safe practice within the domains of:

Clinical judgement and decision making- accountable for providing safe, quality patient care as set by MGH nursing standards of care. Triage for emergent needs and ability for emergent decision making.
Professional relationships- establishes professional relationships with nurses, physicians and other colleagues and patients and their families. Establish close working relationships with the Hepatic Arterial Infusion Pump program team which includes, Physicians, nurses, advanced practice providers and support staff.
Clinical leadership- adherence to MGH nursing and institutional policies and procedures, practice guidelines specific to the setting and maintain ANA Code of Ethics
Clinical scholarship- upholds standards of quality nursing care and supports evidenced - based practice in nursing. Works closely with the scheduling team to ensure patients are scheduled for appropriate postoperative follow-up, medical oncology follow - up, infusion center appointments, nuclear medicine as well as radiology and other in-house services.

The navigator establishes contact with each new and established patient to educate patient and their family members on how to prepare for and to understand the process and expectations of Hepatic Arterial Infusion Pumps. Participates in developing policies, procedures, and standards of patient care in conjunction with the clinic Director and Medical Director. Takes part in developing specific patient care protocols.

Provides for appropriate patient and family education and counseling regarding disease and treatment, potential complications, signs, and symptoms requiring immediate treatment. Documents patient's progress, compliance, and other information in the record.

Participates in ongoing education and networking regarding navigation of patients in the Hepatic Arterial Infusion Pump program. Acts as a liaison between the inpatient unit and ambulatory clinic for collaboration with patient care during admission to the hospital. Scheduling of clinical services in the outpatient and inpatient setting as it relates to the work up, surgical placement, evaluation, and infusion treatments of patients receiving Hepatic Artery Infusion Pump. Serves as a point of contact for MGH clinicians and support service personnel. Participates in interdisciplinary case review meetings and documents consensus recommendations.

Maintenance of patient data tracking logs including referrals, surgical and medical complications, patient outcomes and other relevant clinical data.

Maintains scheduling calendars for various points during the hepatic pump process.

Accompany patients for nuclear medicine SPECT scans to explain procedure and assist in accessing the pump to allow for tracer injection.

Flexibility to assist in hepatic arterial infusion pump accessing and treatment.

Act as point of contact for nursing colleagues in infusion, clinic or inpatient who may have questions, concerns or complications of hepatic pump access. Flexibility to train nursing staff on accessing hepatic arterial infusion pump.

Attends regular tumor boards and team meetings and documents recommendations resulting from meetings in person availability on hepatic arterial infusion pump clinic days and as needed for assistance with clinical needs such as nuclear medicine SPECT scans. There will be the opportunity for remote work based on needs of the program.

Qualifications - External

Must be energetic, organized, and process - focused to coordinate the care of the Hepatic Arterial Infusion Pump patients.
Demonstrates compassion and caring to this specific patient population.
Current Registered Nurse licensure in the State of Massachusetts
Baccalaureate degree required
Oncology nursing experience with solid tumor malignancies preferred
Ambulatory nursing experience preferred
Oncology Nurse Certification required
