About the Role:
Join Tradition Home Health Care as a Home Health Aide (HHA) in Brooklyn, NY, where you will make a meaningful difference in the lives of our clients. We are looking for compassionate and dedicated individuals to provide essential support and care to those in need, ensuring their comfort and well-being.




Responsibilities:

  • Assist clients with daily living activities, including bathing, grooming, and meal preparation.

  • Monitor and report changes in clients' health and well-being to the healthcare team.

  • Provide companionship and emotional support to clients and their families.

  • Help clients with mobility and transportation to appointments or social activities.

  • Maintain a clean and safe environment for clients, adhering to health and safety guidelines.

  • Document care provided and maintain accurate records as required.

  • Collaborate with nurses and other healthcare professionals to deliver comprehensive care.

  • Participate in training and development opportunities to enhance caregiving skills.

Requirements:

  • Current New York State HHA certification or equivalent.

  • Previous experience in home health care or similar caregiving roles preferred.

  • Strong communication and interpersonal skills.

  • Ability to work independently and as part of a team.

  • Compassionate and patient demeanor with a strong commitment to client care.

  • Reliable transportation and willingness to travel to client locations.

  • Basic knowledge of medical terminology and caregiving practices.

  • Background check clearance and current CPR/First Aid certification are a plus.
