Job Description - Home Care Coordinator



Where: 2000 Vermont Dr, Fort Collins, CO 
Pay Range: $19.00 - $21.00 per hour
Hours: Full Time; Monday-Friday 8:00am - 5:00pm; On-call required

We are actively looking for a dynamic person to fill our Care Manager for our Personal Care and Support Services (PCSS) Department. As a Care Manager, here’s a big-picture view of what you’ll do: 



  • Assists the Administrator/Manager to establish and monitor compliance with policies and procedures to ensure client expectations are met or exceeded.

  • Develops, evaluates, and supervises a group of qualified employees.

  • Monitors staff regarding adherence to established standards and procedures for current/new company sponsored systems.

  • Supervises all service operations in compliance with all federal, state, and local laws and as well as all policies, procedures of Interim HealthCare.

  • Coordinates, educates, and contacts prospective and existing clients to assess their needs and reviews the Interim HealthCare full range of service resulting in the achievement of expected goals.

  • Assists with activities of daily living such as bathing, grooming, toileting and elimination and adequate nutritional intake. 

  • Assists with ambulation, transfers and/or range of motion exercises.

  • Assists patient/client with incidental activities of daily living such as shopping, meal preparation, socialization activities, homemaking, and medication reminders.


A few must-haves for our Care Manager: 



  • Minimum of 18 years of age.

  • High school graduate or equivalent and successful completion of a nursing assistant or a home health aide program, or as defined by state law or regulation. 

  • Have experience or training in the home care industry or closely related personal care services.

  • Previous experience supervising employees, preferred.

  • Must be able to work in the field as well as at a desk.

  • Willing to take on-call.
