Home Care Coordinator Assistant

Position Summary

The Home Care Coordinator Assistant supports all functions related to home care coordination. This role requires knowledge of home care products, services, and community resources.

Responsibilities include:

• Explaining service options and helping patients make informed choices


• Coordinating referrals, home health services, and medical equipment needs


• Communicating with agencies and delivering equipment when needed


• Entering data into electronic medical records


• Participating in rounding


• Handling office support tasks such as: 
  
  
  
  • Answering phones
  
  
  • Providing customer service
  
  
  • Ordering supplies
  
  
  • Troubleshooting office equipment and submitting service calls      
  
  
  
  

Qualifications

• High school diploma or equivalent required


• Post-high school education or training preferred


• 1 year of data entry experience


• 1 year of customer service experience in a health care setting


• Previous scheduling experience preferred

Physical Requirements

• Ability to lift and carry medical equipment such as infusion pumps, portable oxygen tanks, or walkers


• Ability to stand, walk, or sit for extended periods

Salary Range: Minimum 16.34/hour - Maximum 26.94/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
