Job Description - Home Care Intake Coordinator

Eminence Home Care is looking for an enthusiastic and compassionate Home Care Intake Coordinator to join our team in Bala Cynwyd, PA. In this pivotal role, you will serve as the initial contact for potential clients and their families, playing a significant part in guiding them through the intake process. You will be responsible for collecting essential information, coordinating care services, and ensuring that the transition into our home care programs is as smooth as possible.

Your excellent communication skills, organizational abilities, and compassionate approach to care will be essential as you work closely with clients, families, and our healthcare team.

Responsibilities:

  • Conduct thorough initial assessments to gather necessary information from clients and their families.
  • Facilitate the intake process, ensuring all documentation is completed accurately and timely.
  • Collaborate with clinical staff to assess needs and determine appropriate service levels for clients.
  • Maintain clear and ongoing communication with clients, families, and the home care team.
  • Ensure compliance with relevant regulations and company policies throughout the intake process.
  • Provide clients with information about services available and support them through the onboarding journey.
  • Previous experience in healthcare or home care intake preferred.
  • Strong communication and interpersonal skills.
  • Organizational skills with attention to detail.
  • Familiarity with electronic medical records and healthcare software.
  • Ability to manage sensitive and confidential information.
  • Compassionate attitude and commitment to quality care.

Hours: Monday to Friday, 9:00 a.m. - 5:00 p.m.

Location: Bala Cynwyd, PA

Salary: $24 - $26 per hour
