What You’ll Do

• 
  

  Perform in-home client visits: initial sign-ups, 30-day check-ins, and 90-day follow-ups.

  
  


• 
  

  Develop and maintain individualized care plans that meet state and federal program requirements.

  
  


• 
  

  Review client rights, responsibilities, and advanced directive plans with clients and/or responsible parties.

  
  


• 
  

  Document visits in WellSky and the state’s EVV system.

  
  


• 
  

  Communicate outcomes with Case Managers and collaborate with Operations Managers on client needs.

  
  


• 
  

  Escalate concerns or complex situations to the Director of Operations for support.

What We’re Looking For

• 
  

  Licensed Practical Nurse (LPN) licensed in South Carolina (required)

  
  


• 
  

  Nursing degree from an accredited program

  
  


• 
  

  Current CPR certification (required)

  
  


• 
  

  Minimum age: 21

  
  


• 
  

  Experience working with older adults, memory care clients, and adults with disabilities or special needs (acute, residential, long-term care, or home care settings)
