Home Care Nurse-RN or LPN

Primary Responsibilities:

• Perform lab draws and medication management support
• Set up and assist with medication organization
• Serve as a liaison between clients and physicians
• Complete patient assessments and accurate documentation
• Supervise Personal Care Assistants (PCAs)
• Support coordinated, patient-centered care plans
• Ensure continuity of care and communication across providers

Qualifications

• Active Registered Nurse (RN) or Practical Nurse (LPN) license required for the state of Minnesota
• Must pass a Minnesota background study
• Compassionate, patient-focused approach to care
