Job Description - Home Care/ Office Coordinator


Comfort Keepers is seeking a highly organized and people-oriented Part time to Full Time Home Care/Office Coordinator to assist with all aspects of Home Care Coordination.  This position is primarily in Cortland NY (Physical Office), it is assisting in Ithaca, Cortland and Binghamton.

Key Responsibilities:
 

           Key Responsibilities:

  • Community Engagement & Marketing:

    • Represent the organization at monthly community meetings and marketing events to enhance brand visibility and foster strategic partnerships.

    • Assist with internal and external marketing efforts to strengthen referral relationships and promote services.

    • Support digital marketing initiatives by creating and scheduling social media content, updating the company website, and participating in online outreach campaigns.

  • Client Intake & Care Coordination:

    • Conduct comprehensive client intakes, gathering essential data and collaborating with families to develop personalized care plans.

    • Coordinate ongoing client care needs, including updates and service modifications based on evolving preferences or conditions.

    • Conduct weekly in home support visits to monitor care quality, ensure client satisfaction, and provide additional support as needed. This ensures compliance with care plans when assigning caregivers, prioritizing compatibility and continuity.

  • Referral & Relationship Management:

    • Cultivate and maintain strong relationships with referral sources such as hospitals, discharge planners, and social workers to support client acquisition and promote services.

  • Scheduling & Operations:

    • Manage and coordinate daily caregiver schedules using WellSky, ensuring consistent and accurate coverage for all clients.

    • Monitor real-time shift activity, responding promptly to last-minute call-outs, cancellations, or caregiver changes.

    • Support day-to-day scheduling logistics and participate in on-call rotation for after-hours coordination (if applicable).

  • Assist HR with Recruiting, Hiring & Onboarding:

    • Assist with recruitment efforts, including posting job openings, screening candidates, and scheduling interviews.

    • Guide new hires through the onboarding process, ensuring all required documentation and forms are submitted to the HR provider.

    • Support caregiver orientation and integration into the schedule.

  • Communication & Customer Service:

    • Serve as a primary point of contact for caregivers, clients, and families—confirming schedules, addressing concerns, and providing timely updates.

    • Uphold high standards of professionalism and customer service through telephone, email, and in-person interactions.

  • Administrative Support:

    • Maintain accurate records of client and caregiver data in WellSky and other systems.

    • Process insurance claims in accordance with company protocols and payer requirements.

    • Provide general administrative support such as answering calls, managing emails, and preparing reports.

  • Compliance & Quality Assurance:

    • Ensure adherence to internal policies and procedures related to client care and caregiver hiring.

    • Collaborate with clinical and management staff to resolve care concerns and enhance service quality.

    • Uphold the organization’s standards of compassionate, high-quality care and service.

Preferred But Not Required: 

  • Experience in recruiting, human resources, healthcare, or customer service preferred.
  • Strong organizational and multitasking skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency in Microsoft Office and scheduling software (WellSky) is a plus.
  • Ability to handle confidential information with discretion.
  • Knowledge of the home care industry is a plus.
  • Reliable transportation required.
