ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

MUST BE A REGISTERED NURSE

MUST HAVE HOME CARE EXPERIENCE

  • · Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
  • · Assist with Home Care, Home Health, and Home Infusion therapy setup and maintenance.
  • · Provides guidance and counseling to coordinators, care managers, and CHWs.
  • · Assists Home Care Aid, Care Managers, and CHWs in managing patients.
  • · Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
  • · Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
  • · Evaluates performance of Clinical Managers.
  • · Assists Clinical Managers to develop skills and techniques in evaluating the performance of clinicians.
  • · Hires, evaluates, and terminates organization personnel.
  • · Conducts clinician performance evaluations annually, or more frequently if indicated.
  • · Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • · Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • · Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
  • · Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
  • · Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel.
  • · Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home care, care management, community supports services provided through the Organization.
  • · Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures.
  • · Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance.
  • · Stays informed about changes in the field of nursing and home care; shares information with appropriate organization personnel.
  • · Promotes home care referrals in the health care community.

MARKETING & SALES

· Strategic Marketing Framework:

· Develop a Strategic Marketing plan following the S.M.A.R.T. criteria, targeting specific demographics and referral networks.

· Identify untapped opportunities and define unique service offerings that set you apart from competitors, focusing on a specialized service aligned with your ethos.

· Deep Understanding of Target Customers:

· Develop insights into target customers, including their pain points and values.

· Identification of Tailored Solutions:

· Analyze and identify tailored solutions for our customer's specific needs, aligning with their unique requirements.

· Mastery of Home Care Marketing and Solution-Selling Techniques: Acquire proficiency in effective marketing strategies and solution-selling techniques tailored to the home care industry.

· Development of a Super Network: Build a robust super network to enhance client acquisition, fostering strong connections within the industry.

· Practice Solution-Selling Sales Calls: Engage in practical exercises to hone skills in solution-selling during sales calls, ensuring the ability to secure advances effectively.

PROMOTIONAL

· Eye-Catching Rack Card: Create visual allure of an eye-catching rack card, a marketing gem that captivates and informs.

· Creation of Website Content: Craft up to two pages of compelling website content that effectively communicates our brand message and offerings.

· Customization of Sales Script Booklet: Tailor a comprehensive sales script booklet to guide our team through effective customer interactions.

· Social Media Content: create topics and quick content for social media posting.

OPERATIONS

Creation of a Simplified Strategic Plan: Develop a concise and actionable strategic plan that aligns with our business goals and market positioning.

Guardian Assistance: Assist with training on proper use of the Guardian System to affiliate, search and dis-affiliate home care aides and employees.

Regulatory Compliance: Assure office regulatory compliance with support and training for Home care compliance and inspection readiness.

QUALIFICATIONS:

1. Registered nurse with current licensure to practice professional nursing in the State of California.

2. Bachelor’s degree in Nursing from an accredited program by the National League for Nursing. Master’s degree in health care preferred.

3. Three years of management experience within the last five years in a home health or related health care organization, at least one year of which was a supervisory or administrative capacity.

4. A registered nurse with four years of experience within the last five years in a home care agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity.

5. Demonstrated ability to supervise and direct professional administrative personnel.

6. Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.

7. Ability to market and deal tactfully with customers and the community.

8. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

9. Has excellent observation, verbal and written communication skills.

10. Knowledge of business management, and governmental regulations.

11. Participation in Quality Assurance/Performance Improvement Program.
