Key Responsibilities 

For Both RN and LPN Positions: 

• Provide high-quality home-based patient care


• Assess patient conditions and monitor health status


• Document patient progress accurately and timely


• Educate patients and family members regarding treatment plans and home safety


• Collaborate with physicians and interdisciplinary healthcare teams


• Support patient recovery, independence, and quality of life 

Additional RN Responsibilities: 

• Perform comprehensive nursing assessments


• Develop, manage, and evaluate patient care plans


• Coordinate care with physicians and healthcare providers


• Supervise LPNs, CNAs, and home health aides as assigned


• Manage patient discharge planning and care transitions 

Additional LPN Responsibilities: 

• Provide skilled nursing care under RN supervision


• Obtain and record vital signs and patient observations


• Assist with implementing the patient's care plan


• Report concerns, changes in condition, and progress to the supervising RN 

Qualifications 

Registered Nurse (RN) 

• Graduate of an accredited nursing program


• Active Florida RN License


• Minimum one (1) year of nursing experience preferred


• Home health experience a plus


• Current CPR Certification


• Strong clinical assessment and communication skills 

Licensed Practical Nurse (LPN) 

• Graduate of an accredited practical nursing program


• Active Florida LPN License


• Minimum 1,200 hours of nursing experience preferred or completion of office preceptor training


• Home health experience a plus


• Current CPR Certification


• Strong clinical and interpersonal skills 

Preferred Qualifications 

• Home health, hospice, or community-based care experience


• Knowledge of state and federal home health regulations


• Ability to work independently while maintaining strong communication with the care team


• Compassionate, patient-focused approach to care
