Home Care RN

University Home Care Inc is seeking compassionate and skilled Registered Nurses (RN) to provide exceptional home care services in Pontiac, MI, and the surrounding areas. As a Home Care RN, you will play a vital role in delivering personalized nursing care to patients in their homes, ensuring their health, comfort, and safety.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.


• Administer medications and treatments as prescribed by physicians.


• Monitor patients' health status and communicate any changes to the healthcare team.


• Educate patients and their families on disease management, medication use, and health maintenance.


• Maintain accurate and up-to-date patient records and documentation.


• Coordinate with interdisciplinary healthcare professionals to ensure continuity of care.


• Provide support and guidance to patients and their families to promote independence and quality of life.

Requirements

• Valid Registered Nurse (RN) license in the state of Michigan.


• Current CPR and BLS certification.


• Minimum of 1 year nursing experience, preferably in home care or related field.


• Strong clinical assessment and critical thinking skills.


• Excellent communication and interpersonal skills.


• Reliable transportation and ability to travel within Pontiac and surrounding areas.


• Ability to work independently with strong organizational skills.
