Position: Home Health Admission RN

Job Summary:

As a Home Health Admission RN, you will be responsible for conducting initial assessments of patients and coordinating their admission to home health services. You will work closely with healthcare providers, patients, and their families to ensure the smooth transition from hospital or facility care to home care.

Key Responsibilities:

- Conduct initial assessments of patients to determine their eligibility for home health services

- Coordinate the admission process for patients, including obtaining necessary medical records and completing required documentation

- Collaborate with healthcare providers to develop individualized care plans for patients

- Educate patients and their families on the services and resources available to them through home health care

- Monitor patients' progress and make necessary adjustments to their care plans

- Communicate effectively with patients, families, and healthcare providers to ensure continuity of care

- Provide direct patient care, including administering medications, wound care, and other treatments as needed

- Document all patient care accurately and timely in accordance with company policies and procedures

- Participate in quality improvement initiatives to ensure the delivery of high-quality, patient-centered care

- Maintain a safe and clean working environment by adhering to infection control and safety protocols

Qualifications:

- Current RN license in the state of Pennsylvania

- Minimum of 1 year of experience in home health or a related field

- Strong assessment and critical thinking skills

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Knowledge of Medicare and Medicaid regulations and guidelines

- Valid driver's license and reliable transportation

- Must pass a background check and drug screening
