Job Title: Start of Care Nurse (RN) – Admissions Specialist

Key Responsibilities:

• Conduct comprehensive Start of Care (SOC) assessments and OASIS documentation
  
• Develop individualized care plans for new clients
  
• Educate patients and families on their care path and available resources
  
• Collaborate with interdisciplinary teams and providers
  

Required Qualifications:

• Active RN license in Minnesota
  
• Proven OASIS documentation and SOC experience
  
• Home health background preferred
  
• Reliable transportation and valid driver’s license
