Position Summary

The Home Health Aide will assist clients in their homes by providing personal care, household assistance, and supporting daily living activities. The ideal candidate is compassionate, reliable, and committed to maintaining the dignity, comfort, and safety of our clients. The Home Health Aide will work under the supervision of a Registered Nurse (or other supervisory personnel) and in collaboration with families to ensure continuity of care.

Key Responsibilities

  • Assist clients with activities of daily living (ADLs), such as bathing, grooming, oral hygiene, dressing, toileting, and mobility (including transfers).

  • Help with light housekeeping tasks (e.g., cleaning, laundry, changing bed linens) to maintain a clean, safe, and healthy home environment.

  • Prepare and assist with meals, including feeding when necessary, following dietary guidelines.

  • Assist clients with medication reminders (as permitted under applicable law/regulation) and monitor their condition for changes, reporting to nursing or supervisory staff.

  • Assist with ambulation and exercise, as prescribed by the care plan.

  • Provide companionship, emotional support and encouragement; help with socialization or engagement in activities based on client’s abilities and interests.

  • Assist with errands, grocery shopping, or appointments as needed and feasible.

  • Document care provided, client’s status, and any incidents or updates; complete timesheets and required paperwork in a timely manner.

  • Adhere to safety protocols, infection control procedures, and all health and safety standards.

  • Maintain client confidentiality and dignity at all times.

Qualifications

  • High school diploma or equivalent (preferred).

  • Certification or training as required by state regulation (e.g. Home Health Aide certification, CPR/First Aid).

  • Previous experience providing home care or personal care assistance preferred.

  • Ability to follow written and verbal instructions, and to work under supervision.

  • Strong interpersonal skills: compassionate, patient, respectful, and able to communicate well with clients, families, and other team members.

  • Physical ability to perform tasks including lifting, bending, transferring clients, walking, standing for extended periods.

  • Reliable transportation and willingness to travel to clients’ homes in the service area.

  • Clear background check, valid driver’s license (if required), and up-to-date immunizations.

Working Conditions

  • Work is performed in clients’ homes, which may vary in condition, temperature, and environment.

  • May involve working evenings, weekends, or holidays depending on client needs.

  • Possible exposure to bodily fluids, disease, and other healthcare-related hazards; PPE provided.
