Position Summary

The Home Health Aide (HHA) is responsible for providing direct personal care and supportive services to patients in accordance with the individualized Plan of Care established by the supervising Registered Nurse or licensed clinician. The HHA plays a vital role in assisting patients with activities of daily living (ADLs), promoting safety, ensuring comfort, and supporting the patients overall well-being within the home setting.

Primary Responsibilities

Patient Care Delivery

• Provide hands-on assistance with bathing, grooming, dressing, toileting, and other ADLs.

• Assist with safe mobility, transferring, and ambulation as directed.

• Support nutritional needs through meal preparation and feeding assistance.

• Maintain a clean, safe, and healthy environment for the patient.

Home Support Functions

• Perform light housekeeping tasks including laundry, linen changes, dishwashing, and home organization.

• Adhere to infection-control protocols and safety guidelines at all times.

Observational & Reporting Functions

• Document care provided during each visit in accordance with agency standards.

• Observe and promptly report changes in patient condition or environment to the supervising RN.

• Maintain confidentiality and compliance with HIPAA regulations.

Professional Conduct

• Demonstrate courtesy, compassion, and respect in all interactions.

• Represent Astra Health with professionalism and integrity at all times.

• Participate in in-service training and competency evaluations as required.

Minimum Qualifications

• Completion of a state-approved Home Health Aide training program meeting Illinois and Medicare requirements.

• Active HHA or CNA certification, as required by regulatory standards.

• Ability to pass criminal background screening and health/immunization requirements.

• Current CPR certification (or willingness to obtain upon hire).

• Reliable transportation, valid drivers license, and liability insurance.

• Ability to travel between patient residences throughout the service area.

Preferred Qualifications

• Prior experience providing home or facility-based personal care.

• Demonstrated ability to work independently while following clinical direction.

• Strong interpersonal and communication skills.

• Multilingual ability is highly valued, as Astra Health serves diverse patient communities.
