Job Description - Home Health Aide II

Job Description:

Job Summary


Responsible for performing non-skilled procedures for home-bound patients. Assists with activities of daily living to support patient comfort and well-being. Ensures a culturally and age-specific environment for care delivery. Follows care plans and communicates patient needs to healthcare teams. Maintains compliance with safety and quality standards.




Essential Functions


• Monitors, reports, and documents patient status and care provided.


• Identifies and recommends changes to improve home safety for patients and caregivers.


• Maintains a clean, safe, and nutritionally appropriate environment in the patient’s residence.


• Delivers personal care using proper procedures.


• Applies correct techniques for patient transfers and ambulation.


• Demonstrates proficiency and accuracy in all home care procedures.


• Performs other duties as assigned.




Education Requirement


Specialized technical, vocational education in the field of healthcare or clinical setting, or equivalent






Experience Requirement


1-2 years of experience in specialized technical, vocational education, or a relevant area




An equivalent combination of education and experience may be considered.


All experience must be paid and in the same related field.


Part-time and PRN experience will be prorated based on hours worked per week.


Volunteer work and internships for academic credit are not counted.




Certifications & Licensures


Current and active Registered Nurse (RN) license, Driver's license

H. Working at heights above 4 feet: 1. Never


I. Working in confined spaces: 2. Occasional (< 10% of the time)


J. Risk of injuries from use of equipment on the job: 2. Occasional (< 10% of the time)

Physical Demands


This position requires regular standing or walking with objects weighing up to 10 pounds; intermittent lifting, pushing, and/or pulling objects up to 50 pounds; intermittent standing or walking with objects up to 25 pounds; occasional lifting, pushing, and/or pulling objects over 50 pounds; and occasional sitting at a computer workstation for extended periods of time. Also involves some exposure to risk or injury from moving, lifting, or positioning patients, equipment, or materials; and exposure to hazardous chemicals and fumes including waste, cuts/punctures, burns, and bloodborne or airborne pathogens.
