Minimum Qualifications

• 
  

  HHA or CNA Certificate (required)

  
  


• 
  

  CPR/BLS Certification

  Level II Background Check (must be clear)

  Negative TB Test (within the past 6 months)

  
  


• 
  

  Physical Exam with statement of being free of communicable diseases (within the past year)

  
  


• 
  

  Valid Driver’s License and Auto Insurance

  
  


• 
  

  Reliable Personal Vehicle (transport may be required as part of the care plan)
