Granite VNA is hiring Home Health and Hospice LNAs! This is a per diem, flexible, job opportunity.

The Home Health and Hospice LNA helps achieve our mission by performing personal care and designated health-related services for Hospice patients in their homes and/or in a facility. The Hospice Community LNA interacts with client's family members, facility staff and other healthcare team members. Some essential functions include:

• Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence
• Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff
• Laundry, housekeeping and maintenance to assure a safe, orderly, clean and comfortable environment
• Provides supervision to patient/residents with cognitive deficit
• Ensures clients have the opportunity to engage in cultural, spiritual, physical, political, social and intelligent activities as the residents’ desire

Requirements

• High school diploma/GED, accompanied by current New Hampshire LNA license
• Reliable automobile and auto insurance
• Valid Driver’s license
• If hired, be available Monday-Friday for the first weeks of employment to complete the required training.
