Are you an experienced Registered Nurse (RN) with a passion for providing exceptional care in a home health setting?
Our fast-growing, dynamic home health agency is seeking an experienced RN Case Manager to join our team. This position offers a chance to be part of an exciting, expanding company with opportunities for advancement, including the possibility of a management position in the near future.
Key Responsibilities:
- Manage and coordinate care for patients in their homes, ensuring the delivery of high-quality, compassionate care.
- Develop, implement, and evaluate individualized care plans.
- Work closely with physicians, families, and the interdisciplinary team to ensure the best patient outcomes.
- Provide clinical assessments, monitoring, and education to patients and their families.
- Supervise home health aides and support staff as needed.
- Ensure compliance with all regulatory requirements and maintain accurate documentation.
Qualifications:
- Active and unrestricted RN license.
- Minimum of 2 years’ experience in home health or case management.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work independently, with excellent time management skills.
- BLS/CPR certification required (other certifications preferred).
- Management experience a plus, but not required.
