Description
The Home & Community Based Services (HCBS) Case Manager is responsible for maintaining and supporting Personal Care Provider (PCP), Home and Community Based Services (HCBS), and In-Home Support Services (IHSS) programs and clients at the direction of The Independence Center (The IC) Home Health Operations Manager or Director of Home Health. This position oversees quality control of the programs, maintains current program clients, and assists with program growth.
