What You'll Do:

• Coordinate and manage patient care plans to ensure high-quality, patient-centered outcomes.


• Conduct comprehensive nursing assessments in the home setting.


• Collaborate with patients, families, physicians, and other healthcare professionals to develop and implement individualized care plans.


• Monitor patient progress and modify care plans as needed.


• Provide skilled nursing services as ordered by the physician.


• Administer medications, treatments, immunizations, and other prescribed interventions.


• Educate patients and caregivers regarding disease management, medications, treatments, and available resources.


• Advocate for patients and help navigate healthcare services.


• Maintain accurate and timely electronic medical record documentation.


• Supervise and collaborate with home health aides and other members of the care team.


• Ensure patient needs are continually assessed and addressed to promote optimal outcomes.

What You'll Bring:

The ideal candidate will be self-motivated, compassionate, organized, and detail-oriented. You must be comfortable with working independently in the field. Our team is passionate about helping patients remain healthy, safe, and independent at home.

• Current Nebraska Registered Nurse (RN) license.


• Current BLS certification.


• Valid Nebraska driver's license.


• Basic computer proficiency, including electronic medical record documentation and patient care coordination.
