Job Description - Home Health Coordinator

Phoenix Home Care and Hospice is seeking a Home Health Coordinator to join our Topeka, Ks team!

As a Home Health Coordinator, you will be responsible for scheduling and coordinating the home health services provided by our dedicated team of nurses. You will work closely with the nursing staff, clients, and their families to ensure efficient and effective care delivery.

Responsibilities:

  • Coordinate and communicate with nursing staff, clients, and their families
  • Maintain accurate records and documentation
  • Troubleshoot scheduling issues and provide timely solutions
  • Ensure compliance with company policies and procedures
  • Assist with other administrative duties as needed
  • Occasionally covering weekend on call shifts

Requirements:

  • Prior experience in scheduling and service coordinating, preferably in a home health or healthcare setting
  • Excellent organizational and time management skills
  • Strong attention to detail
  • Ability to multitask and prioritize effectively
  • Excellent communication and interpersonal skills
  • Proficient computer skills and familiarity with scheduling software
  • Compassionate, caring, and empathetic
