What You'll Do: 

• Serve as the first point of contact for calls, referrals, and general inquiries about home care services
• Coordinate intake process to ensure timely and accurate transition from referral to service start
• Build and maintain positive relationships with referral partners through responsive communication
• Provide administrative and telephone support including scheduling, data entry, and documentation
• Collaborate with internal teams to support recruiting, orientation, and service delivery

What You'll Need: 

• 1-3 years of scheduling or intake coordination experience required
• Age 18 or older with valid driver’s license, vehicle, and auto insurance
• High school diploma or GED required
• Must reside in MN for in office travel
