Job Description - Home Health Intake/Referral Coordinator

Description

Advanced Home Health and Hospice of Arizona is seeking a detail-oriented and compassionate Intake/Referral Coordinator to join our team in Mesa, AZ. This hourly office position involves close collaboration with Home Health and Hospice Clinical Directors to manage incoming referrals from local healthcare facilities and community partners. The ideal candidate should have experience in the field, particularly with Careport, Strata, and Wellsky referral software systems.

Schedule: Full-Time M-F (8:30am - 5pm)

Primary Responsibilities and Duties:

  • Manage new referrals from hospitals, skilled nursing facilities, inpatient rehab facilities, and community partners like primary care physician clinics.
  • Coordinate with Home Health and Hospice Clinical Directors to ensure efficient and timely intake processes.
  • Utilize Careport, Strata, and Wellsky software systems to document and monitor referral information accurately.
  • Maintain strong relationships with referring partners and address any concerns or issues promptly.
  • Ensure that all necessary documentation is obtained and processed according to regulatory and organizational standards.
  • Communicate effectively with patients, families, and healthcare professionals to facilitate a smooth transition into home health or hospice care.
  • Monitor referral trends and provide regular reports to management to support strategic planning and growth initiatives.
  • Collaborate with the clinical team to ensure appropriate placement and assignment of referred patients.
  • Participate in ongoing education and training to remain up-to-date on industry developments and best practices in referral management.

Requirements

Job Qualifications:

  • Experience in home health or hospice referral coordination, with knowledge of industry software systems such as Careport, Strata, and Wellsky.
  • Experience in home health and hospice or hospital/rehab intake and EMR is preferred.
  • Proficiency with desktop computers, medical EMR systems, fax/email, and other relevant technology.
  • Strong phone etiquette, interpersonal, and teamwork skills to foster effective collaboration with internal and external partners.
  • Detail-oriented, organized, and able to manage multiple tasks simultaneously in a fast-paced environment.
  • Excellent communication and customer service skills, demonstrating empathy and understanding when interacting with patients, families, and healthcare professionals.
  • High school diploma or equivalent; associate's degree or higher in a related field preferred.
  • Ability to maintain confidentiality and adhere to HIPAA regulations.

Physical Requirements:

  • Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. 
  • Manual dexterity and hand-eye coordination for handling paperwork, typing, and using office equipment. 
  • Visual and auditory acuity to review documents, communicate effectively, and respond to resident inquiries. 
  • Stamina and physical endurance to handle a fast-paced work environment and meet deadlines for billing submission and account management.
  • Ability to lift and carry boxes of medical records and supplies, as needed, up to 5lbs. 
  • Ability to bend, stoop, kneel, and reach to access files, storage cabinets, and other work-related materials. 
  • Ability to work in an office environment with moderate noise levels and occasional interruptions.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
