Position Summary: Primary role and function is to facilitate patient access to home health by seeking and recruiting referrals for admission. Key to this role is establishing relationships with referral sources, both internal and external, and participating in referral development activities. Adhering to admission criteria and Medicare regulations for appropriate selection, recommendation, and communication for admission to home health. Managing the referral log with appropriate prioritization and communication of patient readiness to all stakeholders. Responsible for referral and admission data tracking and reporting. Completing regulatory documentation to ensure department compliance.
Performs additional tasks and duties to support the Intake office operations. Coordinates with home health leadership on operational functions, participates in department projects and quality improvement initiatives. Works closely with hospital case managers and discharge planners to facilitate patient transition through the continuum of care.
Essential Duties and Responsibilities:
Responsible for screening and accurate completion of referrals to ensure patients meet admission criteria Provide quality customer service; promote collaboration/coordination between referral sources and home health Assess patient appropriateness (initially and ongoing) for home health services and communicates readiness Provide education regarding home health services and conduct referral development activities Verifies home health benefits and obtains authorization for services as indicated Knowledge of Medicare criteria for home health including regulatory and billing standards
Required Qualifications:
Licensed Registered Nurse (IL) 3-4 years’ RN clinical experience with 2 years in home health preferred
