Home Health Licensed Vocational Nurse (LVN) - Simi Valley

About the position:

The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. A licensed vocational nurse shall perform duties consistent with the Vocational Nursing Practice Act. A licensed vocational nurse shall meet qualifications established by the home health agency for the services provided and any additional qualifications required by home health agency licensure regulations.

Schedule: Part Time - 24 RVEs p/week (benefited) 18 RVEs p/week or Per Diem (12 visits per week)
* Non-traditional schedules are available, i.e., Thursday-Monday
Locations: Area #45 covering the cities of Agoura Hills, Calabasas, Moorpark, Newbury Park,  Oak Park, Simi Valley, Thousand Oaks and Westlake Village and Area #42 covering the cities of Canoga Park, Chatsworth, Encino, Lake Balboa, Reseda, Tarzana, West Hills, Winnetka and Woodland Hills
Training: Mon-Fri 9am - 5:30pm onsite at our Woodland Hills office

Minimum Required Qualifications:

• Minimum one year of LVN experience required
• Must possess a CA License with IV/Blood Withdrawal Certification
• Reliable Transportation and auto insurance
• CPR Certification

This is NOT a remote position
