Job Summary:

We are currently seeking a highly skilled and compassionate Home Health LPN to join our team in Milford, PA. The ideal candidate will have a strong background in home health care and a passion for helping patients maintain their independence and quality of life.

Responsibilities:

- Provide skilled nursing care to patients in their homes, including medication management, wound care, and other nursing procedures as needed

- Conduct initial assessments and develop individualized care plans for each patient

- Collaborate with other healthcare professionals to ensure the best possible outcomes for patients

- Educate patients and their families on disease management and self-care techniques

- Monitor and report changes in patients' conditions to the appropriate healthcare team members

- Maintain accurate and up-to-date patient records

- Adhere to all state and federal regulations, as well as company policies and procedures

Qualifications:

- Current LPN license in the state of Pennsylvania

- Minimum of 1 year of experience in home health care

- Strong clinical skills and knowledge of nursing procedures

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Reliable transportation and willingness to travel to patients' homes

- CPR certification

- Ability to pass a background check and drug screening
