Job Summary:

We are seeking a dedicated and experienced Home Health LPN to join our team of healthcare professionals. The LPN will be responsible for providing skilled nursing care to patients in their homes, under the direction of a registered nurse. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for providing exceptional patient care.

Responsibilities:

- Conduct initial and ongoing assessments of patients' health status

- Develop and implement individualized care plans based on patient assessments

- Administer medications and treatments as prescribed by the physician

- Monitor and record patients' vital signs and medical history

- Educate patients and their families on proper self-care techniques and medication management

- Collaborate with interdisciplinary team members to coordinate and provide comprehensive care to patients

- Communicate effectively with physicians, therapists, and other healthcare professionals to ensure continuity of care

- Document all patient care activities accurately and in a timely manner

- Maintain a safe and clean environment for patients and their families

- Adhere to all state and federal regulations and agency policies and procedures

Qualifications:

- Active LPN license in the state of Pennsylvania

- Minimum of 1 year of experience in a home health or similar setting

- Strong clinical skills and knowledge of nursing principles and practices

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and patient-focused attitude

- Reliable transportation and a valid driver's license

- Ability to pass a background check and drug screening
