POSITION SUMMARY
The Licensed Vocational Nurse (LVN) is responsible for delivering essential clinical care to hospice patients during daytime hours. Working closely with the hospice interdisciplinary team, the LVN supports patients with symptom management, medication administration, and health education, ensuring their comfort and well-being in their place of residence.
This is an ideal role for a dependable, compassionate LVN who enjoys fieldwork, patient interaction, and being part of a supportive and mission-driven care team.
PRIMARY RESPONSIBILITIES
- Conduct patient assessments and provide nursing care in accordance with the hospice plan of care.
- Administer medications and treatments per physician orders, following established protocols and safety guidelines.
- Monitor symptoms and report changes in condition to the RN Case Manager or clinical supervisor.
- Educate patients and families on medications, symptom management, and health conditions.
- Document all visits, interventions, and observations accurately in the EMR (e.g., eClinicalWorks).
- Support care for patients with infectious diseases, senior care needs, developmental disabilities, and pediatric conditions when necessary.
- Collaborate with the interdisciplinary hospice team to ensure coordinated and compassionate care.
- Provide in-person care at Fresno Guest Homes and/or patient residences throughout the Fresno area.
QUALIFICATIONS
- Current California LVN license in good standing.
- At least 1 year of experience in home health, hospice, or clinical care preferred.
- Knowledge of aseptic technique and infection control standards.
- Experience with catheter care, tube feeding, and wound care strongly preferred.
- Familiarity with ICD-10 coding and electronic charting systems (e.g., eClinicalWorks) is a plus.
- Strong communication skills and ability to work independently in the field.
- BLS Certification required; IV Certification and ACLS are preferred.
- Must pass a background check in accordance with healthcare regulations.
- Valid CA driver’s license, reliable transportation, and proof of auto insurance required.
- Bilingual (e.g., Spanish, Hmong) is a plus.
Work Schedule
- Friday and Saturday 8:00 AM – 5:00 PM.
- This role will move to Full-Time beginning in May 2026.
