Schedule: Per Diem
Rate: $45-$80 per visit  based on experience and qualifications

What You’ll Do

• 
  

  Provide direct nursing care tailored to each patient's care plan

  
  


• 
  

  Collaborate with a multidisciplinary care team to ensure quality outcomes

  
  


• 
  

  Monitor and document vital signs, treatments, and patient responses

  
  


• 
  

  Educate patients and families on managing conditions at home

  
  


• 
  

  Maintain timely and accurate clinical documentation

  
  


• 
  

  Follow all state regulations and company policies to ensure safe, effective care

  
  

What We’re Looking For

• 
  

  Graduate of an accredited Vocational or Practical Nursing program

  
  


• 
  

  Active LVN license in good standing in California

  
  


• 
  

  At least one year of nursing experience (home health preferred)

  
  


• 
  

  Strong interpersonal skills and ability to build trust with elderly clients and their families

  
  


• 
  

  Able to work independently while following care plans and guidance from the RN

  
  


• 
  

  Physically able to perform job duties including walking, standing, and lifting
