Position Overview:

The Home Health Manager will lead in-home healthcare services for vulnerable seniors, ensuring compassionate, high-quality care that empowers elderly participants to age safely at home. Oversees care delivery across private residences, nursing facilities, and board and care settings while leading, mentoring, and optimizing a dedicated clinical team.

Key Responsibilities:

• Direct home care department operations, ensuring policy adherence and workflow efficiency.
• Hire, train, and mentor home health staff.
• Supervise nursing assessments, interventions, and documentation aligned with care plans.
• Monitor care quality in homes, nursing homes, and board and care facilities.
• Partner with interdisciplinary teams to optimize participant care.
• Evaluate and improve home health service effectiveness.

Qualifications:

• Education: Bachelor’s degree preferred (relevant experience may substitute)
• Licensure: Current California Registered Nurse (RN) license
• Experience: Minimum 2 years in home health (frail/elderly populations preferred)
• Technical Skills: Clinical oversight in home health, nursing homes, and board and care settings, Care plan documentation and compliance, Quality assurance and workflow management
• Soft Skills: Leadership, team training, and coaching, Interdisciplinary collaboration, Complex clinical decision-making for elderly patients
