What You’ll Do

• Provide individual and family counseling in the home setting
• Assist with establishing advanced directives and durable power of attorney
• Facilitate the use of community and home care resources
• Attend and actively participate in interdisciplinary group (IDG) meetings
• Make referrals to long-term care facilities as needed
• Develop and facilitate support groups
• Collaborate with fellow Social Workers and the Hospice team
• Build and maintain professional relationships with staff and referral sources

What We’re Looking For

• Master’s degree in Social Work (MSW)
• Home Health experience preferred
• Ability to work independently and collaboratively
• Professional demeanor and appearance
• Excellent attendance, punctuality, and time management skills
• Strong problem-solving, communication, and multi-tasking abilities
• Positive, professional attitude
