Position Overview:
The Home Health Registered Nurse (RN) provides skilled nursing care to older adults in their homes as part of the Program of All-Inclusive Care for the Elderly (PACE). Working within an interdisciplinary team, this role focuses on delivering comprehensive, patient-centered care to support participants' ability to live safely at home and in the community.
Key Responsibilities:
Conduct comprehensive home health nursing assessments, including evaluating equipment needs and participant preferences and goals.
Participate in Interdisciplinary Team (IDT) meetings to develop and update participant care plans.
Deliver and document nursing interventions as outlined in care plans, including medication administration, tests, and treatments.
Maintain a safe and healthy environment in participants’ homes.
Respond promptly and accurately to physician orders.
Educate participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
Track and monitor home health scheduling in collaboration with the Home Health Manager or designated scheduler.
Collaborate with physicians and other IDT members to ensure smooth transitions of care between settings (e.g., hospital, skilled nursing facility, home).
Qualifications
Education: Bachelor’s Degree required, Graduate of an accredited School of Nursing
Licensure: Unencumbered RN License (California)
Certifications: BLS/CPR + ACLS
Experience: Minimum one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred.
