Position Overview:

The Home Health Registered Nurse (RN) provides skilled nursing care to older adults in their homes as part of the Program of All-Inclusive Care for the Elderly (PACE). Working within an interdisciplinary team, this role focuses on delivering comprehensive, patient-centered care to support participants' ability to live safely at home and in the community.

Key Responsibilities:

• Conduct comprehensive home health nursing assessments, including evaluating equipment needs and participant preferences and goals.
• Participate in Interdisciplinary Team (IDT) meetings to develop and update participant care plans.
• Deliver and document nursing interventions as outlined in care plans, including medication administration, tests, and treatments.
• Maintain a safe and healthy environment in participants’ homes.
• Respond promptly and accurately to physician orders.
• Educate participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
• Track and monitor home health scheduling in collaboration with the Home Health Manager or designated scheduler.
• Collaborate with physicians and other IDT members to ensure smooth transitions of care between settings (e.g., hospital, skilled nursing facility, home).

Qualifications

• Education: Bachelor’s Degree required, Graduate of an accredited School of Nursing
• Licensure: Unencumbered RN License (California)
• Certifications: BLS/CPR + ACLS
• Experience: Minimum one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred.
