In this role, you’ll play a vital part in assessing, coordinating, and delivering high-quality in-home care and skilled nursing services to our PACE program participants. The Home Health Nurse is responsible for assessing the need, monitoring, and delivery of in-home aide services. Responsible for assessing, coordinating, monitoring, and providing health care services and skilled nurse visits for the PACE program participants. The position ensures provision of high-quality care, including coordination with the primary care team.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

The following represents most of the duties performed by the position but is not meant to be all-inclusive or prevent other duties from being assigned when necessary:

  • Participate as a member of the interdisciplinary team (IDT) and initial, semi-annual, unscheduled, and annual assessments; attend morning meeting and report changes in participants’ baseline status to appropriate staff on a daily basis.
  • Plan and coordinate with home care scheduler for adequate staffing of in-home workers.
  • Coordinate service orders with contracted home care agencies, monitor delivery of services, and monitor quality of services provided by home care contractors.
  • Perform in-home skilled nursing services under the direction of a Physician order.
  • Provide follow-up for home care complaints as specified in the LIFE PACE program grievance policies and procedures; document complaints per PACE program grievance policy.
  • Involved in the development and implementation of QI activities.
  • Maintain accurate, detailed reports and records. Record patients' medical information and assessing their conditions and needs in order to develop personalized care plans.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Be flexible in schedule of hours worked.

Requirements

CERTIFICATION, LICENSES, REGISTRATIONS:

Hold and maintains current RN or LPN Nurse license with the State of Oklahoma.

EDUCATION AND EXPERIENCE:

  • Minimum of two (2) years of demonstrated nursing experience; prefer wound care experience.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Effective management skills.
  • Knowledge of utilization review, quality improvement and cost containment systems.
  • Ability to work with ethnically diverse populations.
  • Ability to apply creative problem-solving skills to the complex issues in home care.
  • Ability to work with interdisciplinary teams.
  • Strong organizational skills, flexible, resourceful, and energetic.
  • Effective oral and written communication skills.

WORK ENVIRONMENT: The main work environment is an office setting of a non-profit organization providing a variety of services to seniors and their families. The activity level is high and the noise level is moderate. The work environment will include a variety of office, clinician and individuals’ homes.
