Position Overview

We are seeking an experienced Home Health Nursing Manager (RN) to lead and oversee skilled home healthcare services for older adults living in their homes, as well as coordinate care provided in nursing homes and board-and-care facilities.

The manager will supervise home health nursing staff, ensure high-quality patient care, support interdisciplinary care coordination, monitor clinical quality and compliance, and drive continuous improvement.

Key Responsibilities

  Lead and manage the home health nursing department and daily clinical operations.

  Recruit, train, mentor, and supervise home health nursing staff.

  Ensure nurses provide high-quality care in accordance with individualized care plans.

  Review nursing assessments, documentation, interventions, and patient outcomes.

  Oversee care delivered in patients' homes, nursing homes, and board-and-care facilities.

  Collaborate with physicians, therapists, social workers, and other healthcare professionals.

  Support effective interdisciplinary team (IDT) participation and care planning.

  Monitor clinical quality metrics, regulatory compliance, and performance.

  Lead quality improvement and process improvement initiatives.

  Ensure adherence to organizational policies, clinical standards, and applicable regulations.

  Address clinical, staffing, and operational issues.

  Promote patient-centered care focused on safety, dignity, independence, and quality of life.

  
  

Required Qualifications

• 
  

  Associate Degree required; Bachelor's Degree in Nursing (BSN) or related field preferred.

  
  


• 
  

  Active California Registered Nurse (RN) license required.

  
  


• 
  

  Minimum 2 years of nursing experience in a home health setting required.

  
  


• 
  

  Experience caring for frail, elderly, or medically complex patients.

  
  


• 
  

  Strong clinical assessment and patient-care knowledge.

  
  


• 
  

  Excellent leadership, communication, organizational, and problem-solving skills.

  
  

Preferred Qualifications

• 
  

  1+ year of home health leadership or supervisory experience.

  
  


• 
  

  Experience in geriatric care, skilled nursing, long-term care, or community-based healthcare.

  
  


• 
  

  Experience working within interdisciplinary care teams.

  Knowledge of home health regulations and quality standards.

  Experience supervising or mentoring clinical nursing staff.
