The Home Health Occupational Therapist provides in -home occupational therapy services in accordance with state practice acts, organizational standards, and the Service Agreement. This role focuses on evaluating patient function, developing individualized plans of care, and improving independence, safety, and quality of life in the home setting.
Evaluate patient functional abilities, home environment, and equipment needs; contribute to the overall plan of care
Develop, implement, and adjust treatment plans and functional goals in collaboration with physicians and care team members
Provide therapeutic interventions, including ADL retraining, adaptive equipment instruction, splinting, and prosthetic/orthotic training as appropriate
Educate patients, families, and caregivers on treatment plans and safety strategies
Maintain accurate clinical documentation and communicate changes in patient condition
Coordinate care with interdisciplinary team members and participate in case conferences and in -services
Identify needs for additional home health services and make appropriate referrals
Supervise Certified Occupational Therapy Assistants and home health aides per policy and state regulations
Participate in discharge planning and ensure continuity of care
Graduate of an accredited Occupational Therapy program
Registered Occupational Therapist (OTR) with current state licensure
Minimum of 2 years of clinical experience with at least one year of home health experience (preferred).
Strong communication, documentation, and organizational skills
Current CPR certification
Valid driver’s license, reliable transportation, and required auto insurance
