Responsibilities:

• Perform occupational therapy evaluations and reassessments in the home setting


• Develop and implement individualized treatment plans based on patient needs and physician orders


• Provide therapeutic interventions focused on improving activities of daily living (ADLs), mobility, strength, coordination, and safety


• Educate patients, caregivers, and family members on treatment goals and home exercise programs


• Complete all required documentation accurately and timely


• Collaborate with physicians, nurses, PTs, and interdisciplinary care team members


• Maintain compliance with Medicare and agency guidelines

Qualifications:

• Current and active Florida Occupational Therapist (OT) license


• Minimum of one (1) year of Home Health experience preferred


• Medicare Home Health experience strongly preferred


• Experience with EMR systems (WellSky/Kinnser preferred)


• Strong clinical assessment and communication skills


• Current CPR certification


• Valid driver’s license and reliable transportation
