Home Health Physical Therapist

Are you a compassionate and skilled Physical Therapist looking to make a real impact in patients lives? We are seeking an experienced Physical Therapist to join our dedicated home health team in San Jose, CA. In this role, you'll provide personalized, high-quality care to patients in the comfort of their homes, helping restore mobility, relieve pain, and improve overall function and quality of life.

Key Responsibilities

• Conduct physical therapy assessments, diagnostic tests, and ongoing evaluations for patients under a physician-approved Plan of Care.

• Develop, review, and update individualized care plans in collaboration with the interdisciplinary healthcare team.

• Provide in-home treatments to restore strength, coordination, and body alignment while relieving pain and improving mobility.

• Educate and counsel patients and caregivers on therapy needs, symptom management, and safe mobility practices.

• Document treatment plans, interventions, and patient outcomes in compliance with agency standards.

• Actively participate in care conferences, discharge planning, and quality improvement initiatives.

• Supervise Physical Therapy Assistants and Home Health Aides, ensuring the highest level of patient care.

• Maintain compliance with state, federal, CHAP, and JCAHO regulations.

Qualifications

• Graduate of an accredited Physical Therapy program (APTA-approved).

• Current California Physical Therapist license (or eligibility).

• Minimum of 2 years of healthcare experience (home health experience preferred).

• Current CPR certification.

• Strong communication, documentation, and interpersonal skills.

• Ability to work independently with excellent time management skills.

• Valid drivers license, auto insurance, and reliable transportation.
