Compensation: $100,000–$210,000 annually + $8,000 Retention Bonus

What You'll Do

• Evaluate patients in their homes and develop individualized treatment plans.
  
• Improve strength, balance, mobility, endurance, and functional independence.
  
• Help patients safely navigate their home environments and prevent future injuries.
  
• Educate patients and caregivers to maximize long -term success.
  
• Collaborate with physicians and the interdisciplinary care team to achieve the best outcomes.
  
• Maintain timely and accurate clinical documentation.
  

What We're Looking For

• Active Minnesota Physical Therapist (PT) license.
  
• Strong clinical reasoning and independent decision -making skills.
  
• Passion for delivering patient -centered care.
  
• Excellent communication and relationship -building abilities.
  
• Valid driver's license and reliable transportation.
