Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Home Health Physical Therapist for a growing home health organization in Jacksonville, Florida.

POSITION OVERVIEW
• Deliver one-on-one physical therapy services in patients' homes
• Manage your own schedule with flexible weekly planning
• Evaluate patients, complete start-of-care visits, reassessments, and discharges
• Zoned territories help minimize travel time
• Stable patient census with strong long-term earning potential

COMPENSATION
• $55-$65 Per Unit
• Guaranteed Minimum Of 24 Units Per Week
• Most Physical Therapists Average 30-35 Units Weekly

SCHEDULE
• Full-Time
• Flexible Self-Managed Schedule
• No Structured Call Requirements

ROLE
• Perform comprehensive home health physical therapy evaluations
• Complete start-of-care assessments, reassessments, and discharge visits
• Develop individualized treatment plans
• Deliver patient-centered rehabilitation services in the home
• Maintain accurate clinical documentation using WellSky/Kinser EMR
